Achieving Success with a Physician Led ACO

Our presentation will describe the processes that we employed to start a physician led Accountable Care Organization. We performed an environment survey to understand the current costs and trends in our community. We then organized a collection of independent primary care practices that shared the same philosophical goals and were performing at the level of a medical home. We will describe initiation of care management; Use of analytics in selecting consultants, hospitals, home health agencies; implementation of a health information exchange to reduce waste and duplication; collection of quality measure data for reporting to CMS or commercial insurance companies. Finally we will outline our latest deployment of “closed loop” analytics to allow physicians to close gaps in care.
